HOW TO CREATE A NEW AUDIT
QRM (Quality Risk Management) is a system that allows organizations to schedule and complete audits in real time or on demand. It supports KPI tracking and the development of quality improvement plans using real-time data.
QRM includes pre-built audits that can be used as is, customized, or created from scratch. Audits may be set up as a Standard Audit (completed by one person) or a Part Audit (completed by multiple individuals).

To create a new QRM:
1. QRM can be accessed by clicking on the QRM icon from the main menu located on the left hand side.
2. Click Manage QRM
3. Click Create QRM
The new QRM can be copied from an already existing audit. To copy select the checkbox next to the COPY QRM FORM
4. Type in the QRM Name
5. Select or Create the QRM Category
6. Select or create the audit indicator
7. Add the instructions or upload from an existing document
If this is a confidential audit (i.e. employee performance review), select the Yes option.
8. Click Submit
The name, instructions, and indicator are all saved, edits can be made by clicking the edit button at the top of the form.
You must add a Part, and then a Section to begin adding the questions. Parts and Sections can be added on continuously as you create your audit.
Once you have added your section/part, you can then begin to add and edit questions. Questions can be added individually or in bulk (import) questions.
Adding Individual Questions
- Add the question
- Select the question type (Radio dial, check box, text box, Date, Slider, Number, dropdown and users)
- Option Template for the Radio Dial question (answer template, yes/no; true false)
- Add Information Text (allows you to add extra direction, steps for the individual completing the observation/audit)
- Select the allow comment, allow pictures, allow document (if applicable)
- Type in the option answers
- Select Deficient Answer if applicable
- Add/Creator Indictor
Bulk Adding Questions to QRM
All questions copy over as radio button options, and must be edited to fit the criteria.
Question Type Options
The answer options include:
- Check box (select all that apply)
- Date
- Date & Time
- Radio Button
- Slider and Text
If a trigger survey is required, select the applicable option and then you can create it, type in the survey title, or use the drop down arrow to search for the trigger audit.
Additional options after you have added a question
Once you have added your question, you can:
- Add additional questions
- Add skip logic to the question (skip to the next applicable question)
- Copy, Edit or Delete the question
Adding Skip Logic
Skip logic is a feature that automatically directs respondents to relevant questions based on their previous answers, allowing them to skip questions.
The steps to adding skip logic:
1. Click on the Add Skip Logic button
2. Select the Section within the PARTusing the drop down arrow that you would like to skip to,
3. Select the question using the drop down arrow.
4. Click Submit.
Adding/Editing Additional Parts
Additional Parts can be added/copied/moved from any of the other parts.
You can move the entire part by selecting the drop down arrow and selecting the part you would like it moved to.
The double page icon allows you to copy the entire part.
The plus sign icon allows you to add a new part.
The pencil icon allows you to edit the current part.
The X icon allows you to delete the part.
How to Add/Edit/Move a Section
Additional Parts can be added/copied/moved from any of the other parts.
You can move the entire part by selecting the drop down arrow and selecting the part you would like it moved to.
