HOW TO ASSIGN A SURVEY
INTRODUCTION
The Survey tool can be used to create questionnaires or feedback forms, that can either be anonymous or not anonymous depending on the nature of the feedback you are collecting.
All survey’s must be activated before publishing. Activating the survey is completed from the Pending Publish Repository.
To activate your survey:
- Locate the survey in the Pending Publish Survey
- click the applicable check box
- Click activate from the top menu bar
TO ASSIGN A SURVEY
- Click SURVEYS from your side bar menu.
- Click SURVEY LIBRARY.
- Click the checkbox for the survey title you would like to assign and click ASSIGN
The survey name will appear at the top of the screen. In the example below, the survey is “Employee Satisfaction”. From this screen you can:
- Click ADD MORE to add additional surveys to this assignment
- Click the X to remove any titles that you no longer wish to assign
- Click SUBMIT AND CONTINUE to proceed
There are 3 types of assignments that can be selected, they are:
Open Surveys – allows you to set a time frame where the survey will be available to staff.
Mandatory – allows you to make this survey mandatory for staff by entering a due date for completion.
On Demand Surveys – Allows the survey to be launched numerous times (as needed).
For example: If an employee is required to go room to room to gather feedback for Resident/Client satisfaction, they can launch a new survey each time they meet with someone.

After you have specified the type of survey (Open, or Mandatory), set the frequency for its completion.
You can select a specific education group.
Assign the survey to specific users by searching for their name.
You can exempt specific people who might be in a group who don’t need to do it.
To make the survey not anonymous, you can override the “survey details” setting by selecting YES.
- Click SAVE to finish assigning the survey.
