HOW TO CREATE A NEW SURVEY
INTRODUCTION
The survey functionality is a great tool to use to help gather information.
Some of the features included when creating a survey include:
- They can be created so they are anonymous or have it identifiable (user name will appear).
- A survey from scratch, or you can copy an existing survey and make changes as required (i.e. delete/add questions).
- Surveys can be sent with due dates to specific education groups, outside members of the facility (i.e. board members, family members) or they can be set ‘on demand’, meaning it will be available for an indefinite period.
- Skip logic can be applied to questions. If a certain question is answered, you can automatically have the next most applicable question come up for the user to complete.
- Trigger Survey functionality can be added. If a certain question is answered, you can automatically have the applicable survey come up for the user to complete.
To Create a New Survey:
- Click SURVEYS from the side menu bar.
- Click ADD NEW SURVEY
- Type in the applicable details, which include:
- Survey Title
- Add the organization logo
- Select the Category the Survey belongs in using the dropdown arrow
- Type the instructions
- Upload additional instructions (i.e PDF)
- Using the radio dial you can choose to have the survey anonymous
- Click save
- Click Add a Section Header
- Type in Section Header Title
- Click Save
You have options to either add a question, import questions (from a word document). You can also edit your section name, or delete it.
- Click on the applicable option
Question Type Options
The answer options include:
- Check box (select all that apply)
- Date
- Date & Time
- Radio Button
- Slider and Text
If a trigger survey is required, select the applicable option and then you can create it, type in the survey title, or use the drop down arrow to search for the trigger audit.
Additional options after you have added a question
Once you have added your question, you can:
- Add additional questions
- Add skip logic to the question (skip to the next applicable question)
- Copy, Edit or Delete the question
Click Save, once you have finished creating your survey.
Your survey will be saved into the Pending Publish Repository until you are ready to assign.
